I need to email a PHP Form to myself...Help!

I have no clue about programming or writing scripts which is why I buy them. The one I bought recently has a series of scripts to make up a booking system. For some reason the developer (and I use that term loosely) did not think it was necessary to have a new appointment that is scheduled online send an email to anyone other than the end user. I need it to also alert me via email of the new appointment. Any help is appreciated. Attached is the form used by the visitor to make an appointment. Hopefully I can just add a line or two.

Oops! Sorry about that. That's a different script in the same program and that's a line that I stuck in there thinking it would work but when I put my email address in there I get an error that an unexpected '@' is there. I am attaching the correct form script. Thanks or the response.

I think confirmation.php is needed for us to determine how to modify the script to fit your needs. There is no email sending in timebook.php, just the data collecting.
