Well what is your site hosting? It sounds like something on your site code is wrong.

The server has only 1 site of news, with 5k daily visitors
484 requests currently being processed, 0 idle workers
The same IP generated more than 400 request and the server crashed
Normal apache is:
28 requests currently being processed, 2 idle workers

Seems you have to block that IP; do you have any firewall applications installed ?

You need to check the size of each images/data in your website and the site loading time using http://www.websiteoptimization.com/services/analyze/
What is your domain name ?
Also how much RAM you have in the server ? What is the maxclient setting in httpd.conf
