We have been having this issue for a while now and have been searching for answers so this is our last resort. We had a customer of which requested this so it was installed however we receive 1 of 2 errors. Error 1: When attempting to create a database and/or username - we get this error: Error 2: When attempting to create a database and/or username - it will act just like it went through successfully however when clicking return, everything is blank as if it was never created. Any help would be SO appreciated as this is getting rather irritating trying to figure out.

There has to be more to the error than what you're showing. Are there log files generated by whatever tool you're using to create the user or DB?

Any logs would help in narrowing down the issue. Meanwhile , try this : Go to /var/lib/pgsql/data/pg_hba.conf Change ‘md5′ to ‘trust’. It should look like :

Well we did a complete uninstall then a new install - that fixed that problem but now a new one has occured. It will not log in when going to the phppgadmin, it just says login failed on the accounts that I have tried - even our own. What passwords do I need to change, the account password or is there something that I am missing?
